In the remote mountains of Colorado, horrors wait inside Mount Massive Asylum. A long-abandoned home for the mentally ill, recently re-opened by the “research and charity” branch of the transnational Murkoff Corporation, the asylum has been operating in strict secrecy… until now. It has been revealed that a sequel is in development but red Barrels have stated that they will not release a game unless they themselves are afraid to play it
The concept for the game came from Aphex Twin's "Rubber Johnny" video.

The main characters entire name "Miles" is a reference to the term "miles up shore without a paddle", meaning that he is in too deep to return.
As Miles' name was never revealed to other characters, each of them assigns him with a different name, some of them being "little pig" by Walker, "buddy" by Trager and "my son" by Martin.
It was originally planned for Miles to have been sexually assaulted at some point in the game. This was later scrapped by the developers as they've considered the scene to be excessive.Quotes

In the small shed in the courtyard, the outline of a chainsaw on a shelf can be seen. This is a reference to the movie Evil Dead 2, in which the main character, Ash, reaches for a chainsaw in a shed, only to find an outline of it instead.
